MAT Application Process

  1. Review the Qualification Criteria

    To qualify for State-provided equipment, a MAT program participant must:

    • Be a Maryland resident age 3 or older who has difficulty using a standard phone. (Applicants under 18 require a parent/guardian signature.)
    • Be certified as Deaf, hard of hearing, DeafBlind, blind, low vision, or having cognitive, mobility, or speech difficulties.
    • Be on a fixed income or receiving state/federal benefits (SSA, VA, SSI, SSDI, TDAP, TANF, or pharmacy, medical, or housing assistance).

  2. Complete the Application

    You can apply online or download an application from our website. Want an application mailed to you? ​ Call 800-552-7724 or 410-767-6960 (Voice/TTY) or 443-453-5970 (Video Phone) to request an application by mail. Spanish-language applications - PDF are also available.

  3. Get the Application Certified

    Online: Digitally sign and submit your application directly—no mailing required.

    Mail In Application: Mail or take the application’s Disability Certification Form (labeled Part 4) to one of the following licensed professionals for certification:

    Licensed physician \ Audiologist \ Speech Pathologist​ \ Rehabilitation counselor

    This person, acting within the scope of his or her license, will certify that you are unable to use a standard telephone, and that you can benefit from this program.

  4. Enclose the Proper Documentation

    • Online Sign-Off: Digitally sign through the portal (no mailing needed).
    • Proof of Income/Benefits (Provide ONE):
      • SSA / SSI / SSDI: Copy of SSA Award Letter
      • Benefits: Proof of VA, TDAP, TANF, or pharmacy/medical/housing assistance
      • Income: Proof of fixed or limited income
    • ID & Proof of Address (REQUIRED):
      • Photo ID
      • Telephone or Utility Bill

  5. Read, Sign and Mail All Forms

    (Integrated into online application - Digitally sign your application, No mailing required)

    Incomplete or unsigned forms will delay processing. Applicants under 18 require a parent/guardian signature.

After You Apply

Once your completed application is received, it will be reviewed by the MAT program for approval. If you are approved, you will receive an acceptance letter that includes information on scheduling your evaluation. The goal of the evaluation is to find the equipment options that meet your needs. Equipment will be mailed to you 4-6 weeks after your completed evaluation.
